Immunization Requirements

• In order to protect children from common serious childhood illnesses, the State of Arizona has joined all other states in requiring a series of immunizations for entry into school at various levels.

  Parents are required to provide the school with their child's immunization record upon entry into school. If your child has not completed the full series of the required vaccinations, please arrange for this as soon as possible.

  *For a child entering Kindergarten, 5 DTP/DTaP/DT are recommended. A child who has received the fourth dose after the 4th birthday will meet the requirement

  **If your child who received the 3rd dose of Polio after the 4th birthday, no further doses are required.

  *** The first MMR vaccine must be after the first birthday
